.To register for the Symposium, please complete the on-line REGISTRATION FORM.

Please note that registration procedure has 2 steps: first fill in your Personal data form, then continue with Registration form, choose the fee, the term of payment and finish the form. After submitting the filled form you will be provided with the Letter of Confirmation and the button for „online payment“ will be available. Kindly note that the payment by online system is available only after the Letter of Confirmation is issued. The Letter of Confirmation will be issued within 1-3 days (usually in one day).

Please note that on-line Registration Form is accessible only with a password.

REGISTRATION FEES AND DEADLINES

Authors	deadline for ADVANCED REGISTRATION was postponed to December 8th. From December 1, 2008 the on-line registration form is set up for late registration fee, please, put down a note "abstract" when you submitted abstract and we will change your registration fee to early bird.

*Students must attach a copy of their student ID, and Trainees a signed certificate from their department head or supervisor.

**Registration fees for all Accompanying persons, no matter from which area, do not include attendance to any of the Symposium Scientific Sessions.

WHAT IS COVERED BY THE FEE?

Registration fee for Delegates and Students covers:

Attending all Symposium Scientific Sessions
Participation in Welcome Cocktail
Admission to the exhibition area
Symposium materials
Coffee breaks

Registration fee for Accompanying Persons covers:

Participation in Welcome Reception
Prague City sightseeing tour
Admission to the exhibition area

CONFIRMATION OF REGISTRATION

If your completed Registration Form and payment or proof of payment is received by the Symposium Secretariat before 15 January 2009, you will receive a written confirmation of your registration by e-mail. Registration Forms received after 15 January 2009 will not be handled and confirmed, participants have to register on-site in hotel Diplomat (Symposium venue).

Please present the Letter of confirmation at the Registration desk at the Symposium to pick up your materials.

Please note the deadline 30 November 2008 to benefit from the reduced rates. Registration Forms without payment cannot be processed. Early registration fees will only be confirmed if the Registration Form and the payment are received by the Symposium Secretariat on 30 November 2008 at the latest.

CANCELLATIONS, CHANGES AND REFUNDS

Please inform the Symposium Secretariat of any amendments or cancellations in writing by fax (+420 284 001 448) or e-mail. Telephone amendments will not be accepted. The appropriate refunds will be made after the Symposium.

The following cancellation conditions apply to the cancellation of a registration:

Cancellation before and on October 31, 2008	full refund less a handling fee of 50 EUR
Cancellation from November 1, 2008	no refund

TERMS OF PAYMENT:

Bank Transfer

All bank details for the payment are mentioned in the Letter of Confirmation.
Please note that all bank charges must be fully covered by the participant.
A copy of the bank transfer must be sent together with the Registration Form.

Credit Cards

Mastercard/Eurocard, VISA, Diners Club and American Express credit cards are accepted only.

For the payment by credit card kindly use Online payment system (available after login). The payment by online system is available only after the Letter of Confirmation is issued. You can also use completed Credit Card Payment Authorization Form (doc) signed by a card holder. Kindly fax it to the fax number +420 284 001 448.

According to the Czech law, credit cards will be charged in local currency – Czech crowns (CZK) by using the exchange rate of the Czech National Bank on the date of payment. The approximate exchange rate is 1 EUR = 24 CZK.

Name change

If you should be prevented from attending, you will be given the opportunity to send a colleague in your place. Name changes should be notified in writing to the Symposium Secretariat. A handling fee of 30 EUR will be charged for each name change.

PASSPORT AND VISA REQUIREMENTS

Visitors from non-EU Countries must posses a passport valid for at least the next 3 months. Participants requiring a visa should apply immediately to a consular office or a diplomatic mission of the Czech Republic in their country in order to avoid a delayed arrival to the Symposium. Please note that the visa application procedure can take up to two months. It is not possible to obtain visa at the Prague Airport.
The Symposium Administration assists the delegates by processing the official invitations on request.
These letters do not commit the Local Organising Committee to any financial obligation.

Contact the embassy and ask which of the following documents they require to issue a visa for you for the Symposium:

Official invitation by Symposium Organisers or Official invitation by the Foreigners' Department of the Czech Police.

As soon as you know which of the above documents you need, send an e-mail to the Symposium Administration (c4c@guarant.cz) with information needed to issue the invitation.

Official invitation by the Foreigners' Department of the Czech Police – information needed:

The official invitation issued by the Foreign Nationals Department of the Czech Police costs 95 EUR (delivery service included).

Official invitation by Symposium Organisers or Official invitation by the Foreigners' Department of the Czech Police will be issued after registration to the Symposium and payment of the registration fee.

As soon as the required data are received – no later than December 1, 2008, the Symposium Administration will start the procedure of issuing the official invitation for you. The procedure usually takes a few weeks. When the official invitation is ready, it will be sent to you by post or express delivery service. You shall then present it at the Czech embassy in your country and get visa for the Symposium.
